When Your Nervous System Won't Turn Off

If you've had insomnia, you know it's not about being lazy or unable to relax. Your nervous system has learned to treat sleep as a threat that requires vigilance and problem-solving. Your body is doing exactly what it's designed to do—stay alert. It's just doing it at the wrong time.
This happens through classical conditioning. A few bad nights, and your brain pairs the bed with anxiety and wakefulness. Over time, just walking into the bedroom can trigger the arousal response—elevated heart rate, muscle tension, racing thoughts—the exact opposite of what sleep needs.
The Neuroscience of Staying Awake
Your cortisol (the stress hormone that normally peaks in the morning) stays elevated at night, suppressing melatonin. Your circadian rhythm—the 24-hour clock that tells your body when to sleep—flattens or shifts. Your core body temperature fails to drop at bedtime. Your reticular activating system, which keeps your brain alert, stays chronically upregulated. Your amygdala (threat detector) is hyperaware. The regions that should turn sleep on turn quieter instead.
Conditioned arousal adds another layer. Environmental cues—the bedroom, the pillow, the thought of bedtime—become triggers. Your body begins releasing cortisol before you even get to bed, anticipating the fight you've learned to expect.
This is not a willpower problem. This is a nervous system that has become extremely skilled at the wrong job.
Why This Matters for How We Treat It
Understanding the mechanism changes everything about treatment. If insomnia were just a deficit—"you can't sleep," like you can't see without glasses—the answer would be a pill or willpower. But hyperarousal is learnable. And what's learned can be unlearned. Therapy has a fix - and a modified CBT-I can help with neurodivergent individuals... That's where we start.
CBT-I: Retraining the Nervous System
CBT-I (Cognitive Behavioral Therapy for Insomnia) works by breaking the conditioned association between bed and arousal. Instead of trying to relax harder or think different thoughts, CBT-I addresses the nervous system directly through behavior change. The core mechanisms are simple: rebuild the bed as a safe place for sleep (not a place where you've failed), restore a natural sleep drive by consolidating sleep, and reset circadian rhythms through consistent timing. It's not about being "perfect." It's about creating the conditions where sleep can happen again.
Standard CBT-I typically includes sleep restriction (compressing time in bed to match actual sleep), stimulus control (bed is only for sleep and intimacy, not worry or screens), sleep hygiene adjustments, and cognitive work around catastrophic thinking about sleep. These are evidence-based and they work. But they were developed on insomniacs without ADHD, autism, or other neurodivergent wiring. Standard protocols assume your executive function is intact, your sensory environment is easy to control, and your internal sense of time and fatigue is reliable. For many neurodivergent adults, that's not accurate.
So we modify. We meet the nervous system where it is while still using the core logic of CBT-I. Sleep restriction becomes more flexible and graduated—sudden compression can dysregulate ADHD or autistic nervous systems further. Stimulus control accounts for sensory needs: maybe the bedroom needs specific lighting or temperature regulation that neurotypical protocols don't mention. Cognitive work shifts from fighting intrusive thoughts (often an exhausting dead-end for neurodivergent minds) toward recognizing thoughts without the demand to change them. We build in accountability structures and external time cues when internal awareness is weak.
